it feels like the choice of starting gender and background changes more than just the start and a few one off lines of dialogue. im seeing lines in one route that i really dont remember from the first time i played

Honestly, I kinda like that he's not even a bad guy - just... not remotely suited for the position he's in, and unprepared to properly perform the role.
It's a weird, but satisfying ending to have him just bail and go find himself IMO. Poor guy got shoved into the worst harem manga plotline possible (half a dozen girls competing for your affection while you save the world, but you CAN'T HAVE ANY SEX AT ALL.) and he never even had the chance to realize he's probably not into girls at all.
Is there a trigger for continuing the story or is it just when enough time passes? I have done every single hangout so I'm not sure if there's hidden objectives or anything.

Is it just me or does ophelia's 5th hangout always seems to break the game? idk if it's because i started game shortly before update or is it just the way it is
I came back to go play this game again after being gone from a few updates just so I can finish this game's story. I saw your comment before I started the game and so I went through Ophelia's hangouts to see if the 5th one would break my game. Surprisingly, it didn't so I'm not sure what answer to give yah.
